2 報名表:個人的感覺沒有什麼需要特別注意的,按照要求如實填寫就行了。沒有的比如ssn添none即可。
3 指紋卡:新墨郵寄指紋卡,OR MAKE REQUEST
(http://www.fbi.gov/hq/cjisd/forms/description-fd258.htm)。在市公安局作的指紋,按照指示做就行,印完之後還要他簽名(在指紋卡相應位置)然後在他的簽名上蓋上公章。記得做兩份。一共100塊。
4 fingerprint certification form: 這個也沒有什麼好說的,按照要求添就行了。和指紋卡一起讓簽字及蓋章。
5 nationwide criminal background check:需要公正的只有這個——簽名公證!之前要把這張紙上的內容自己翻譯一下列印出來,因為公證處要留底, 當著公證員的面把名字簽上(當然是簽拼音不是漢字阿)然後給他,三天後這張紙會被訂在一份五頁的公證書中。需要說明的時公證處不會直接在這張紙上蓋章但是不要擔心,公證書上的鋼印已經是有效的了。一份200元, 至於attention以下的部分自己添添就行。
6 匯票:如果有護照就可以直接拿美金買匯票,沒有護照的就只能先用人民幣購匯再辦理。記得一定要帶本人身份證,還有各個地區規定不同,這邊要收手續費20,也是三個工作日之後才能拿到,在目的地地址處添New Mexico Department of Public Safety。
7 payment form:就是160美金,用visa卡。
8 中英文成績單:是從學校直接發出的,其他沒有什麼好說的,就是學校蓋章的正式成績單。耐心仔細閱讀那些instructions,查詢哪裡辦指紋卡哪裡做公證而已。
9 重審核,指紋卡有效期已過。第一次差不多用三四個月的時間。nm現在的辦事效率很低,重審核,沒有牽涉到任何證件的補辦,也等了近2個半月,要重審指紋卡,那麼最少應該也要3個月了
10 表格上有一項是:DECLARATION OF PRIMARY STATE OF RESIDENCE IS MANDATORY FOR LICENSURE (原始居住州聲明),還用黃線加重。就填你所報名的那個州,以後轉執照還可以變更。

Frequently Asked Questions about Fingerprinting and Endorsements
Q : Why do I need to be fingerprinted?
A : The Nursing Practice Act 61-3-13 and 61-3-18 requires that applicants for initial licensure or endorsement, at their cost provide the board with fingerprints and other information necessary for a state and national criminal background check.
Q : Where do my fingerprints go?
A : Your fingerprints are submitted to the New Mexico Department of Public Safety for a statewide criminal background check and also to the Federal Bureau of Investigation for a nationwide criminal background check.
Q : Who can take my fingerprints?
A. Public law enforcement official or other agency staff trained by the New Mexico Department of Public safety or an equivalent state agency in another state. In a foreign country, public law enforcement official or other agency staff who have been trained how to do fingerprinting.
Q. Do I need to send the fingerprints with my application?
A. Yes, the application will not be processed without submission of fingerprints.
Q. Can I submit one money order for both my application and fingerprints?
A. No, one money order or cashier's check made out to the New Mexico Board of Nursing is for your application and the other money order or cashier's check made out to the New Mexico Department of Public Safety is for the state and nationwide background check.
Q. Where do I get fingerprint cards?
A. You can request them from the board in writing, by phone or e-mail boardofnursing@state.nm.us . You can also use cards that are issued by the FBI. They must be lined with blue on the front and have the following on the back of the card-Federal Bureau of Investigation United States Department of Justice CJIS Division/ Clarksburg WV 26306. Leave the following boxes blank if the cards are not issued by the board of nursing: ORI and reason fingerprinted.
Q. How long does it take to complete a background check?
A. The background check can take up to 12 weeks or more if fingerprints are clear and classifiable. If information is missing, fingerprints are rejected. If the fee and/or fingerprint certificate form, and/or Authorization for Release of Information are not submitted, all can be sent back and you will be asked to resubmit all required documents.
Q. Can a permit to practice or temporary license be issued while the background check is being completed?
A. Yes, if your file contains the necessary documentation.
Q. Can a license be issued while the background check is being completed?
A. Yes, if you file is complete.
Q. If I was fingerprinted by another agency or board, can these results be shared with the New Mexico Board of Nursing?
A. No, the FBI does not allow this and each agency has different criteria that may disqualify an applicant.
Q. What criminal action would be cause for a disciplinary hearing before the board?
A. A criminal history of conviction of felonies or violations of the Nursing Practice Act.
Q. What should I do if I have been charged with or convicted (including a nolo contendere plea or guilty plea) of a felony (or criminal offense) in any state or in federal court (other than minor traffic violations) whether or not sentence was imposed or suspended.
A . You will need to provide the board with a certified copy of the court records regarding your conviction, the nature of the offense, date of discharge, if applicable, as well as a statement from the probation or parole officer.
Q. What should I do if I have had any discipline action pending against me by any licensing jurisdiction, the USDA, Drug Enforcement Agency or any state drug enforcement authority?
A. You will need to provide the board with a certified copy of any documentation which needs to include where and when the action occurred.
Q. Who can I speak with if I have additional questions?
A. Any staff member should be able to answer your questions or you can ask to speak to the person responsible for finger printing.
ENDORSEMENT
Q . Is there a different form for a Temporary License?
A . No. The endorsement application has an area where you can request a temporary license.
Q . Is there an addition fee for the Temporary license?
A . Yes. The fee is listed on the endorsement application.
Q . How long does it take to receive a Temporary License?
A . There is a processing time of up to 2 weeks.
Q . Can I have more than one other state license at a time?
A . Yes, if the other state license if from a non-compact state. If you are endorsing from a compact state and you are declaring NM as your home state, please contact the compact state you are coming from to let them know that you are endorsing into NM. If you are maintaining residence in the other compact state, that license will be recognized in NM and you will not need to endorse. If at a later time you decide to become a NM residence, you will need to endorse into NM and notify the other state what you are doing.
Q . Can I endorse into New Mexico being a Foreign Graduate?
A . Yes, if you hold a license in another state or US Territory.
Q. Which form do I use to send for verification, NURSYS or endorsement request form?
A . Submit NURSYS form only if your original state of licensure is listed on the form instructions. The endorsement request form is for verification for all other states.
Q . I am an Advanced Practice Nurse; do I only send in the endorsement application?
A. No. You must also include an endorsement application for advance practice with your endorsement application. |
